Abstract: This article presents my work with a patient whose sense of Self was profoundly unformed, necessitating extensive engagement within the realm of State 2 maladaptive affective experience. An AEDP’s guiding ethos – to create our interventions, moment-to- moment, by tracking the unfolding phenomenology – allowed me to innovate within the AEDP framework. In the course of my work with this client I devised two new triangles of experience: the Triangle of Finding and Rescuing the Self, and the Triangle of The Emerging Self. These triangles offer a conceptual visualization of the work primarily happening within maladaptive State 2 affective experience and in the process of moving from maladaptive affective experience into adaptive affective experience.

The theoretical framework in this article builds on Eileen Russell’s (2021) incorporation of agency, will, and desire as State 2 affective experiences. She proposes the idea that “neglect, exploitation, and oppression of any kind leave holes in the development of self” (p.244), a deficit that Fosha has similarly described as unformed experience (Fosha, 2013). I propose that when working with clients who present with an unformed Self, we must connect with our patient within the realm of State 2 maladaptive affect; it is there that we encounter the neglected split-off part of the Self, needing to be rescued. As was true for my patient, the maladaptive affective experience might be the (yet) only “road” to their inner emotional life. Without being recognized, witnessed, and felt in the presence of a safe ‘other’ the split-off divided self cannot be unified, and the True Self cannot form and transform. Simply said, the patient needs to know they have a Self and have a felt sense of that Self before they can feel about and for the Self.

Introduction

This article draws on a decade of experience using AEDP when working with the deepest forms of attachment trauma, specifically, the trauma of deep neglect. This type of trauma results in a state of emotional isolation bereft of a felt sense of Self, of other, and of existing as oneself in the world. In the emotionally isolated patient, though a patient is estranged from a sense of self, this Self does exist. This True Self resides as the neurobiological core self (Fosha, 2013; 2021), but for relationally traumatized clients it is out of reach. This is because the neurobiological core self can only be accessed and felt in the context of relational safety;

that is, in a relationship with a “safe enough” attached other. In the state of emotional isolation a part of the patient’s self, or sometimes the entire self, exists in a state of shutdown, with little to no ability to reach out, seek safety or comfort from others, or receive any help that may be offered.

An essential part of the therapeutic process, therefore, is about creating a space where the patient feels safe enough to allow core aspects of the self to emerge. Fosha (2013) states that the relational safety must be employed in the “search-and-rescue mission for and of the patient’s felt sense of self.” We engage to “find” glimmers of the hidden self, encourage and support these glimmers, and then gradually help the patient be able to take in, and hold onto, the new experience of feeling their Self. It’s a slow process that requires repetition and patience.

This article illustrates how the attachment-informed therapeutic stance in AEDP (Fosha, 2000), combined with the approach that allows moment-to-moment phenomenology to guide the process, creates the interactive space necessary for the development of the Self. This process inevitably leads to reactivation of very early attachment needs which is when engaged with therapeutically an opportunity for healing. In the case featured below, this reactivation took form at the level of the early adaptive function of imitation, one of the most important interactive features for learning and survival (Meltzoff, 2017).

Three vignettes from my work with a patient, whom I refer to as Ellie1, are used to illustrate the therapeutic process involved in finding glimmers of Ellie’s Self helping her to experience and reclaim her True Self. The various challenges encountered in the therapeutic process with Ellie required of me, as the therapist, to deepen and refine my understanding, as well as further develop my AEDP stance and skills in working with clients who have minimal access to a sense of Self. Crucial to my understanding of Ellie’s therapeutic needs – of how to work therapeutic with what is ‘not there,’ is Eileen Russell’s (2021) conceptualization of the agentic self as a core affective state. She proposes that the sense of agency, will, and desire are themselves core affective experiences AND they may be absent in our clients when “neglect, exploitation, and oppression of any kind leave holes in the development of self” (Russell, 2021).

The work with Ellie led to my development of a new, deepened, and expanded area of intervention within State 2 work, in order to specifically address what was missing – her undeveloped sense of agency. The two new “triangles of experience” I developed will be used to help track the process and evolution of Ellie’s growth.

It was important to first create an expanded space for the agentic Self to be identified, then felt, and filled in and formed. Creating and sustaining this expanded space is challenging for

therapists when working with clients such as Ellie, for whom the diminished sense of self and agency impairs their engagement in the therapeutic process – in Ellie’s case, she could not speak. Such fine-tuning of presence and enhanced listening is beneficial even for patients less traumatized than those I refer to in this article. Such ‘doing’ of AEDP, requires ‘being’ present for the patient (Lipton, 2021), right brain-to-right brain, and moment-to-moment tracking that experience, balanced with left-brain understanding. It is a bottom-up, inside-out learning that builds trust in the therapeutic process. Moreover, it greatly promotes agency – our innate capacities to be who we are, as well as to genuinely be together.

My work with Ellie was critically informed by AEDP’s clinical application of infant research. AEDP plays a significant role in the “bridging” between infant research and adult treatment and is especially helpful in working with patients such as Ellie, who suffer with a debilitating undeveloped sense of self. I explore and illustrate how AEDP’s therapeutic stance, devised from observations of secure attachment promoting parent-infant interactions, enhanced Ellie’s capacity to engage in the therapeutic process and establish therapeutic trust. This stance, together with tracking the phenomenology of the patient’s moment-to-moment experience, can guide the therapeutic dyad on the journey that begins from emotional isolation with no felt sense of Self and arrive at the felt sense of existing as oneself, and as oneself in-the-world with others.

PART IEllie: working without words

There is nothing such as an isolated individual. Organisms survive only in relation to an environment, influencing that environment and in turn being influenced by it (Regina Pally, 2005, p. 208).

At some point, early in the therapy, Ellie became mute. She reverted to just nodding (“saying yes”) or shaking her head (“saying no”) in response to my words. Of necessity, we needed to begin there, in order to “find” her. Years later, she described, “I needed the nonverbal thing. My words felt false, but that which came from my body felt right. I wasn ́t in contact with the words AND my body. When I talked, it felt like the contact was broken.”

I never experienced Ellie ́s muteness as a defensive withdrawal. Intuitively, I felt there was something “right” in her turning mute. Although silent, it was intense, and I felt her desire to connect through her nodding or the shaking of her head. She was present, there, on the edge of what was possible in that moment, but it was something more than that. The muteness seemed like an entrance to her true Core Self, an invitation to BE with HER. A fragile NEW space. It was her way of being ME/I with YOU from a place of awakened healing and self- righting drive.

Diana Fosha says, “Lodged deeply in our brains and bodies lie dispositions for healing and self-righting that surface under conditions of safety; they are there for the awakening from the first moments of the first session onward” (Fosha, 2021, p.28). The “mute part” in the process with Ellie was her nonverbal response to safe enough conditions with me, and it was the beginning of the journey to rescue and support the emergence of her Self.

Attachment trauma and the emotionally isolated Self

Working with attachment trauma and neglect (errors of omission), and the emotionally isolated Self – the result of the trauma (Fosha, 2000), has always been an area of interest to me. AEDP opened a new arena and landscape for this kind of work and developed a new platform for me to work from.

It is slow work and requires patience, from both the therapist and the patient. I see the need to deepen our understanding of these processes and to develop our ways of working therapeutically with this group of patients. Understandably, therapists can easily get lost in the quietness/silence stemming from the patient ́s lack of experience with self and shutdown states, when “facial expressions are masked, and words are withheld behind walls of silence” (Pando-Mars, 2016, p.9); however, this may result in unintentionally leaving the patient alone with the painful fear that there is no way out of their emotional isolation.

Right-brain-to-right-brain communication

Ellie suffered from deep neglect in her early life and childhood. She never experienced the ”nonverbal thing” in a way that allowed her to sufficiently develop and grow. Yet, she knew she needed “the nonverbal thing.” She explained that what came from her body felt right. Her need embodied what infant researchers have discovered about the earliest forms of interaction: that the bodily aspect of communication is an essential component of the capacity to communicate and understand emotion (Beebe & Lachman, 2014). Beebe & Lachman (2014), highlight how the nonverbal interaction plays a key role in attachment formation and they draw a link to adult treatment, noting that “nonverbal communication is remarkably similar across the lifespan.”

While Ellie knew she needed the nonverbal ”thing,” and it felt right. I, as her therapist, also sensed something right in her becoming mute. AEDP therapists are trained to be on the lookout for glimmers of transformance – the innate motivational drive to heal, self-right, and “strive toward maximal vitality, authenticity, and genuine contact” (Fosha, 2008, p. 282; 2017a). This involves seeing the patient through an adaptive lens rather than a pathological one (Fosha, 2000). This knowledge informed my approach to Ellie’s muteness and allowed me the freedom to lean into my own right-brain sensing and knowing, thus attuning to her glimmers of healing and self-righting, rather than feeling stuck or puzzled by her silence. I

was able to remain open to “learning the nonverbal, moment-to-moment rhythmic structures of my patient’s internal states” (Schore, 2016, in Foreword to Marks-Tarlow, p. 13).

My work with Ellie emerged through a place of “no words and no content.” The AEDP approach and stance helped me stay in a kind of extended nonverbal openness and listening beyond what I was used to, and I developed an increased tolerance for “the tension between knowing and not knowing.” (Lipton, 2021, p.149). Schore emphasizes that right-brain-to- right-brain communication is critical for the development of the self and for the regulation of emotions throughout life (Schore, 2019). He discusses how unresolved trauma, particularly early relational trauma, is stored in the right brain and can lead to various forms of psychopathology. He argues that effective therapy must address these trauma-based, right- brain processes to promote healing. He also discusses the importance of therapists attuning to their own right-brain processes to enhance their effectiveness. (Schore, 2012; Schore, 2019).

Recognition

Ellie needed my “independent subjectivity,” my recognition and the experience of mutual influence (Jessica Benjamin, 2005), all crucial aspects for early self-development. Tronick introduced the concept of the ‘dyadic expansion of consciousness,’ where both the infant and caregiver co-create emotional and cognitive states through their interactions and where both grow and develop as a result of their exchange. He highlighted how this mutual regulation of emotions is foundational to the development of the self (Tronick, 2007).

Although Ellie deeply needed the experience of mutual regulation and mutual influence, this was also something that, in her adaptation to such severe neglect, she had learned to stop needing. She both lacked a reference inside herself for these kinds of experiences and simultaneously feared the new experience. This condition turned out to be one of the greatest challenges in our work together.

The findings of the Still-Face Experiment (Tronick et al, 1978) are relevant to our work with patients suffering attachment trauma. This well-known psychological study conducted by Dr. Edward Tronick in the 1970s, examines the infants’ reaction to emotional and social disconnection from their caregivers. In the first stage of the experiment, the caregiver (usually the mother) and the infant sit facing each other. The mother is first instructed to interact with her infant as she normally would – smiling, talking, and responding to the baby’s cues. In the next stage, the mother is asked to stop responding to the infant and instead maintain a neutral, expressionless “still face” while continuing to look at the child. Then, after 2 minutes of “still face” the caregiver is instructed to return to normal. The infant’s profound response to the mother’s sudden emotional unresponsiveness while in “still face,” embodies the excruciating experience of ‘nobody there.’

In the early 2000s, at a workshop in Sweden, the infant researcher George Downing compared the “still face” response between securely attached and insecurely attached infants. The securely attached infants reacted with the well-known pattern: surprise—trying to get the mother’s interest back—distress/anger—sadness—despair—panic/fear while the insecurely attached infants did not react, typically lowering their gaze to their own lap. The insecurely attached infants had already learned to protect themselves from the excruciating panic/fear response that the emotional unavailability of the mother gives rise to. This explains the fear- based risk aversion that neglected patients have for taking in new experiences with another.

Along with the developing right-brain-to-right-brain presence with Ellie, addressing the significant therapeutic challenges and fears also required a developed left-brain understanding on my part, informed by infant research, which became increasingly essential as new phenomena emerged.

A phenomenon that occurred late in the process with Ellie further contributed to my understanding of the profound importance of nonverbal communication in the development of the Self. While reviewing a video-taped session, I noticed that Ellie imitated me in the way infants do. It was a spontaneous, unconscious mimicry of my body movements.

Agency, will and desire – the missing link

Even though the degree of Ellie’s pathogenic shame was reduced and felt safety with me was deepened, she nevertheless struggled to hold onto these new secure attachment experiences, as if she didn’t know how to do it. On this matter, I resonated with Russell’s (2021) observation that “something essential is missing from the sense of self that is not simply going to emerge fully formed, even in the safest, most secure therapeutic relationship.” This insight provided me with a crucial comprehension of Ellie’s lack of agency, will, and connection to her desire, and moreover that the missing link in our work – the missing aspect of her Self, must be centered. In Part III I will describe how we went about recovering and developing this missing aspect of her Self. But first, I would like to review a set of key concepts in AEDP (Part III picks up on page 12).

Part IIOverview of AEDP theory and methodology

6 Key concepts in AEDP

1. AEDP is a non-pathologizing model that works with trauma and attachment trauma (Fosha, 2000, 2021), from a fundamental understanding of psychopathology as a result of aloneness in the face of overwhelming emotions and as the by-product of internal working models born out of insecure attachment experiences. The AEDP therapist’s first task is to foster secure attachment from the get-go, and to employ interventions to undo this kind of unbearable

aloneness. Through its attachment-based approach, AEDP “places the therapeutic relationship front and center in its mission to heal relational trauma and transform the Self” (Piliero, 2021, p. 269). The AEDP therapist ́s genuine affective engagement and responsiveness is crucial, and the stance brings forward the therapist ́s deep warmth and genuine care for the patient.

2. AEDP has a strong foundation in developmental research and research on transformational affective change processes. From this knowing, the “doing” of the therapy is based on the moment-to-moment tracking of the phenomenological manifestations of affective experience, in the patient, in the therapist, and in the interaction between patient and therapist (Fosha, 2021). This approach not only serves as a foundation for developing secure attachment, but also encourages mutual openness for exploration, trust in the process, and creates a space for the patient to discover and build trust in their own innate (unbroken) capacity to heal and grow. The moment-to-moment tracking helps to identify “subtle and not so subtle nonverbal cues of affective shifts” (Hanakawa, 2021, p.107) in the patient, therapist, and dyad. These phenomenological shifts are what therapists base their interventions on.

3. AEDP is oriented toward healing, guided by the foundational principle that healing is an innate, wired-in, biological process, ever present in all of us (Fosha, 2021). Transformance is AEDP ́s term for the innate motivational drive to heal, self-right, and flourish (Fosha, 2008, 2017a). The concept of ‘honoring defenses’ is crucial to the therapeutic process, involving the recognition and respect for the adaptive functions that the patient’s psychological defenses have served. This approach together with the understanding that there exists in all humans a fundamentally integrative neurobiological core self (Fosha, 2013, 2021) are important guiding concepts in the work of helping the deeply neglected patient come out of their emotional isolation.

4. AEDP fundamentally focuses on experience. It is a mind-body therapy that works “bottom- up,” with an intensive focus on present-moment, internal experience, and the felt sense of affective experiences as they arise in the body during the here-and-now of the therapeutic encounter. In doing so, the AEDP therapist strives to “make the implicit explicit and the explicit experiential” (Fosha, 2021, p. 33).

5. The use of the therapist’s Self (Lipton, 2021) together with the focus on “making the implicit explicit and the explicit experiential” are key interventions in the work of helping the neglected patient take in/receive new experiences.

6. A key component of AEDP methodology is its innovation of Metatherapeutic processing, or metaprocessing – the processing of positive affects or changes for the better. It helps the patient solidify the change they have experienced. A frequent refrain in AEDP trainings is simply, “First, you must have the experience, but then, you must also know that you have had the experience.” When working with deeply neglected patients, we can add, ‘and, you must be able to take in the new experience.’

The phenomenology of the four-state transformational process

The process of healing and change unfolds through four distinct states, each with its own unique phenomenology. In AEDP, these have been mapped and tracked to guide the therapist—and is known as the phenomenology of the four-state transformational process (Fosha, 2009). I will illustrate each of the four states with Ellie’s process.

Patients often enter therapy in State 1, connected to their suffering. Often, the emotionally isolated patient enters therapy with no connection to either their suffering or to who they are. Only a vague yet pervasive sense of not being well.

In State 1, side by side with the suffering and its manifestations, there are manifestations of transformance. Transformance is described by Fosha (2008), as “the overarching motivational force, operating both in development and therapy, that strives toward maximal vitality, authenticity, and genuine contact” (p 292). These manifestations or glimmers of transformance indicate that the client’s inherent drive for healing and growth is already at work. The therapist’s role is to help nurture and amplify these transformance glimmers to facilitate a softening in the defenses or an opening to the isolation, which can lead to a State 2 experience.

In the process with Ellie, I intuitively sensed that her “muteness” – instead of being a sign of resistance or pathology – was one of those glimmers of transformance.

State 2: Adaptive Core Affective Experiences AND Maladaptive Affective Experiences

State 2 refers to the phenomenon that occurs when defenses are lowered, allowing the patient to access their affective experiences. State two consists of two categories of affective experience – core adaptive experiences and maladaptive experiences.

Core adaptive experiences are: “categorical emotions of sadness, anger, fear, joy, and disgust; coordinated relational experiences; receptive affective experiences; intra-relational experiences and their associated emotions; attachment strivings; somatic ‘drop-down’ states; experience of agency, will and desire; and authentic self-states” (Fosha, 2021, p. 44).

Maladaptive emotional experiences, such as toxic shame, fear, and unbearable aloneness, lack adaptive action tendencies, cannot be processed, and are not inherently transformative. They require transformation, which sometimes leads therapists to leave the maladaptive field of experience too soon. However, I want to emphasize that the maladaptive experience is still crucial – it holds the neglected truth and a split-off part of the Self, and the maladaptive experience might, for the neglected patient, be the (yet) only ”road” to their inner emotional life. Without being recognized, witnessed, and felt in the presence of a safe Other, the split-off divided self cannot be unified, and the True Self cannot emerge. This is where the journey— from ”no self to core self”- must begin. As SueAnne Piliero describes it, “It’s here, in these deep, dark, shame- and terror-filled, warded-off places within the human psyche, where the deepest healing needs to take place” (Piliero, 2021, p.274).

Intra-relational Parts work is a vital therapeutic intervention when working in the realm of State 2, particularly when working with maladaptive affective experiences. Parts, or child parts, are fragmented aspects of the Self that haven’t been integrated due to deficiencies in early attachment with caregivers. This approach helps the patient develop a new understanding of themselves, recognizing that their feelings and beliefs about themselves or others can stem from old memory feelings (parts) of unprocessed (and/or unformed) experiences, and discern the old from what is happening in the here and now.

The understanding of “parts” is crucial in the work of ‘finding’ the wounded Self. The wounded Self often emerges as an image of a younger Self, ‘the little me,’ as it did for Ellie, who found her wounded Self as a dirt-covered, barely alive little girl in the mud. When given the space, the creative and resilient right brain can begin to work its magic, showing us what happened (the wound), and what needs to happen (to heal the wound and develop the emergent Self). Androff and Russell (in preparation) term these spontaneous images “emergent imagery” and describe how they can occur “unscripted.” Emergent imagery that occurs within right brain-to-right brain communication is an entry into both affect and the unconscious.

In the session transcripts I will demonstrate my use of these techniques. The transcripts will demonstrate the necessity to engage in extended work with State 2 maladaptive affect with Ellie before she could enter the transformational and healing realm of State 2 adaptive affective experience. Simply said, the patient needs to know they have a Self, and have a felt sense of that Self, before they can feel about, and for, the Self.

State 3: Transformational Experiences (feelings of mastery, pride, mourning the Self) and State 4: CORE STATE and The Truth Sense

State 3 is characterized by the emergence of the transformational affects associated with experiences of healing, transformation and/or change for the better (Fosha, 2021, p 45). It ́s about taking in, deepening and expanding the experience of change or healing that the patient just went through.

State 4 is the phenomena emerging when the patient finally has access to the true core self The fears, doubts or forces against accessing the True Self have been transformed. This state is characterized by a deep sense of calm, clarity, and coherence within the Self.

Introducing new triangles and expanding State 2 workState 2 work between maladaptive affective and adaptive affective experience

AEDP is a model, not a method. The work is grounded in the moment-to-moment tracking of fluctuations in the experience of the patient, dyad, therapist, and process. We track the felt sense of affective experiences as they arise in the body in the here-and-now of the therapeutic encounter. The therapist uses these fluctuations to guide the choice of intervention (Fosha, 2021, p 31). I want to emphasize the importance of this focus on following the patient and the process. Nothing more effectively enhances the patient’s sense of agency than the experience of being continuously responded to. One of my patients, when reflecting back on the process, exclaimed: “It (the model) really followed…WE followed my body. Not even I knew…it just emerged kind of. It feels like such a beautiful model – to be able to follow from inside what you didn ́t even know existed.”

According to the same principle, the AEDP therapist uses the four-state transformational process as a map, not a manual (Kranz, 2021), where the phenomena guide and orient the therapist as to whether they are on the right track with any given intervention. These distinctions make AEDP an open space for development and exploration. It was by following the phenomena that occurred during these processes, that I was able to identify some new patterns that enhanced my understanding of attachment trauma and of what can be developed within the therapeutic relationship. The patterns occurred in State 2, between maladaptive and adaptive experience. Between, because it happened in a state of openness to new experience (”Self at best”), not in maladaptive affective experience, but still many steps to go before reaching core adaptive affective experience.

The new triangles:

Through tracking the phenomena that occurred, along with my own and my patients’ experiences of the process, two triangles of experience gradually emerged that correspond to two phases of this expanded work in State 2.

PHASE 1. The Triangle of Finding and Rescuing the (Wounded) Self. ”Do I exist?”

In this phase of the process, glimmers of the truth are unfolding. The patient begins to recognize and accept that ‘something happened (or didn’t happen) to me,’ followed by the realization that the neglect did not reflect upon the patient, but upon the parents’ deficit – in the words of a patient, “It wasn’t me who couldn’t form an attachment; it was them, my parents, who couldn’t.” The patient also begins to understand that the initial false self- attribution of fault or deficiency was a necessary survival measure to protect the attachment bond to an insufficient caregiver. These realizations are necessary for the patient to begin to fill in the missing parts of self. Delineating a separation of Self from the qualities of the caregiving one received (or didn’t) is, in and of itself, an undoing of pathogenic shame.

The work is about tolerating the previously unbearable truth – of ‘nobody there’, and/or, of having been used as a surrogate (Russell, 2021). It’s a beginning step in the process of separating the True Self from the neglectful caregivers. The survival strategy – to preserve the attachment bond at the expense of the True Self – was necessary and adaptive back then, but in the here and now, it prevents the individual from being who they are, by compromising the inborn ability to feel, communicate, and connect (Frederick, 2021, p 190).

This part of the process entails “feeling the holes.” As one patient described: ”It was like an unfolding, it was painful, about finding the wound. I didn ́t know where we were going, and it went on in the darkness.” Even though the patient can be processing deep core state experiences and having deep insights about their wounds and its consequences, they may not necessarily be solidly in the core affective adaptive area of experience. They may be close to

isolation.

For example, one patient had an experience of seeing straight into the ”split” inside. She stated, “It ́s actually about a splitting. I had to split myself. Split-off the truth. I never saw it like that before.” She saw the split-off part of her truth while feeling the deep pain of what the splitting had done to her Self. The realization of the splitting marked the beginning of ability to her bring the parts of the self back together. This moment of realization gave rise to a profound core state experience, wherein she declared, “ I saw my soul!” Although being in a deep and clear Core State experience, there was no release of adaptive action tendencies and no emergence of resilience. This was a glimmer (or sparkle) of her True Self, but she still had one foot in the emotional isolation while beginning to sense the first vulnerable feeling of truly existing in her own right.

PHASE 2: The Emerging Self. ”Who am I?”

The patient is now ready to take the first steps out of emotional isolation and experiences glimpses of a new felt sense of being free to be who they are. The focus of therapy is now about “birthing,” developing and exploring the emergent Self. The patient who described the ”unfolding” in phase 1 describes her experience in phase 2: “It was like the same unfolding, I didn ́t know where we were going, but it was now going on in lightness”. The process is about “filling the holes” and forming the unformed experience.

Once the felt sense of existing and the right to be who one truly is, has been sufficiently integrated, there is a shift in the process when the patient finally enters the adaptive affective field of experience. These experiences are now transformative, and the processing of (adaptive) emotions to completion can begin. Different patients have similarly described this shift with words like: “This is the first time I experience that what we are doing is healing.” They describe emergent experiences of resilience, and they start to report a sudden emergence of memories of (traumatic) situations and relationships that previously lacked meaning. But now, when seen through “new eyes” these memories become deeply clarifying and further explain the depth of the wound and the emotional isolation. AND through the new access to their core emotions and the truth of what happened to them, the old trauma memories can now be processed to completion.

PART IVEllie: working within the new triangles – between maladaptive and adaptive experience

At the beginning of therapy, Ellie was intensely focused on not losing my attention. She wasn’t yet ready to risk feeling anything within herself. However, beneath her focus on me, in the moment-to-moment tracking, I began to notice a more subtle reaction from Ellie—barely noticeable moments of freezing or going silent in response to my mis-attunements or misunderstandings2. Through my recognition of and taking responsibility for these ruptures (micro-ruptures), Ellie slowly began to develop a budding sense of having boundaries that were recognized and respected. This marked a beginning in the ”filling the holes” in the development of her Self.

With this growing, yet fragile, sense of a boundaried Self, she could start to articulate what her freezing or going silent was about. She explained, “She (referring to her child part) gave up, she didn’t feel understood.” Together, we realized that, at this stage of development, Ellie needed me to understand her perfectly. Although her adult part understood that perfect understanding was impossible, she couldn’t stop her automatic disengagement, or “giving- up,” when she felt misunderstood. Her need in this early phase of the development of her Self resonates with the amazing quote from Winnicott in a 1949 paper, “…. in the development of every individual, the mind has a root, perhaps its most important root, in the need of the individual, at the core of the self, for a perfect environment” (Winnicott, 1949, p. 247).

In the same paper Winnicott also claims that the need for a good environment, which is absolute at first, quickly becomes relative. The ordinary good mother is good enough. If she is good enough, the infant becomes capable of compensating for her deficiencies through mental

Finding, forming and transforming the Self Medbo, A. 17 activity or understanding and develops a growing tolerance in relation to both “ego needs and

instinctual tension” (Winnicott, 1949, p 246).

Ellie’s need to be perfectly understood and her declaration of “giving up” was an act of “mental activity and understanding” from the core of her SELF, marking the first step in developing her sense of self, separate from me/the other. It was an early form of differentiation, saying: “I have a need that isn’t understood by YOU.” A first experience of being *I* with you, rather than an I submerged in “you”, or a differentiated true “we”.

All these “giving-ups,” combined with my active efforts to reconnect us, formed the essential foundation for the development that later took place. Ellie needed these repairs, “in the countless moments of interaction,” to “fill” in one of the earliest holes in the development of her Self (Tronick & Gold 2020, p. 39). Tronick & Gold found that without the opportunity to move through disruption to repair in early interactions with caregivers, infants develop behaviors consistent with sadness, withdrawal, or disengagement. They also observed that these infants seemed to struggle with holding themselves together – “either their movements were disorganized, or they collapsed and became very still. Both responses suggested that they felt helpless and ineffective.” These findings explain Ellie’s fear of losing my attention (as if losing my attention meant “there would be no way back”) and her later “giving-ups,” which stemmed from a place of having no expectation of repair. But/AND the ”giving-ups” were, in themselves, a development of her ability to stay present in her own experience and a new step of differentiating her Self in the relationship with me. And they didn ́t become toxic because I took them as signs for the need for repair.

When Ellie finally reached the adaptive affective field of experience, she explained in a session: “I ́ve realized that I ́ve started to tolerate the fact that you don ́t understand me perfectly.” Beyond being an incredible testament to her growth, it also clarified what we had been doing and why. It embodied two of the three organizing principles for development of attachment described by Beebe & Lachman (2000) – ongoing regulations, and disruption and repair of ongoing regulations. The third organizing principle: heightened affective moments (Beebe and Lachman, 2002), later became integral to the process of building the Self as Ellie’s receptive affective capacity increased.

PHASE 1. Working in The Triangle of Finding and Rescuing the Self: two case vignettes

Case vignette 1: Meeting patient at Phase 1; Finding and Rescuing the Self phase, glimmers of core, spontaneous Self.

When I first meet Ellie, she is in her late 20s. She is seeking therapy due to a profound sense of not belonging and a fear of social situations. She hasn’t been able to complete any studies and recently resigned from her latest job due to her social anxiety. Ellie can’t share much about her past, other than that her parents divorced when she was 8 years old. Her mother

struggled to cope with the divorce, and when her father met a new woman, her mother became deeply depressed. Ellie mentions that her mother is emotionally cold and rejecting, and her father emotionally distant, with little capacity to be present for his daughter.

This vignette illustrates the first part of the process, the finding and rescuing of the Self. Ellie is still not connected to her suffering, nor to who she is. There are glimmers of contact with her core Self but with frequent slips into maladaptive patterns. Ellie alternates between withdrawal and struggling to stay connected. Shame and fear easily take over. She is deeply afraid of feeling anything within herself. Although she has come out of her muteness, it is still difficult for her to talk, to stay with, and share her own experiences. We will see how challenging it is for her to allow anything to emerge from within herself, and how terrifying and shame-inducing it is. However, by holding the space and inviting her back, and with her effort to stay present both in herself and with me, we eventually reach some precious moments of contact with her core, spontaneous self.

We will observe how my intention and persistent focus is on encouraging any part of HER to come forward, while also holding space for the difficult experiences. It’s important to note that I recognize both the glimmers of emergence AND the painful truth of what is holding her back—the lived but unfelt reality of nobody there, of not being seen or heard, and the experience of her BEING as something dangerous. I want to emphasize that my acknowledgment and holding of these two aspects of her Self—the wounded Self and her True Self, and the ”unbearable pain that lives between the two” (Piliero, slides, 2020)—are what finally allow Ellie, even for a brief moment, to feel seen, not alone, and safe enough to allow a glimmer of her emergent Self to appear.

For an extended period, the work continues in this manner. In retrospect, I understand that we were engaging in ongoing regulations, and rupture and repair of ongoing regulations. Ellie needed me to guide her back to herself repeatedly, over and over again.

She still can’t share much about her childhood, but there are two scenes we can use to gain a better understanding of what happened – and what “didn’t happen” – to her. One scene involves the 8 years old Ellie, who, frightened by a dream in the middle of the night, asks her mother if she can get into her bed. She is allowed to get into the bed, but she senses the terrifying truth, that her mother is not psychologically present; she feels completely alone in the world. The other scene involves the 10-year-old Ellie, who often fantasized about being adopted, riding her little bike to a woman in the neighborhood who made her feel seen. One day, when she returns home from her bike trip to the neighbor, she is met by her father, who says, “What are you doing? The neighbor woman doesn’t want kids running around in her garden.”

This experience left Ellie in a state of deep shame, which not only prevented her from seeking attachment with the woman in the neighborhood but also stopped her from fantasizing about being adopted. The shame kept her from going against her father, in order to “maintain the (secure) connection with her primary caregiver at all costs” (Frederick, 2021, p. 192). In therapy with me, she gradually began to recognize and accept these behaviors and fantasies as

adaptive strivings and a form of transformative strength. The shame she felt for longing for different parents is slowly transforming into a new understanding – an acknowledgment of both the deep lack of emotionally available parents, and the strong inherent drive of the little girl to keep seeking what she intuitively knew she needed.

This understanding opens the door to her wounded self. In a profound session, she finds her inner child, represented as a dirty figure, barely alive in the mud. In deep pain, she expresses, “I never realized I was this damaged.” This experience leads to a profound sense of relief. Discovering her wounded self allowed her to feel ”found” – meaning, ”maybe I do exist.”

Through working with her younger parts, we begin to distinguish the present from the past and separate the child part from the adult part. Ellie needs my support and deep care, although it is still difficult for her to fully accept it. But I trust the process. Two years into the therapy, she says, ”This is the first time I hear you,” and a year later, she dares to look me in the eyes for the first time. Even though she cannot verbally respond to my attempts to be there for her, I understand that she IS receiving my listening and acknowledgment.

Levels of undoing aloneness – pushing with connection

The only way to come out of emotional isolation is through relationship. There is a beautiful quote from Alan Sroufe, “Before there is a self, there is relationship… and it is from that relationship that the self emerges” (ISC International Conference, 2021. David Elliott). Through the work presented in this article we could add or make a distinction, saying: “In the beginning there is a neurobiological core self (Fosha, 2000, 2021) in a need of a perfect environment (Winnicott, 1957)….and it ́s (then) from the influencing and being influenced by a good enough environment (Pally, 2005) that the FELT SENSE of a differentiated whole self emerges.” Working relationally “tends to reactivate insecure attachment learnings” (Frederick, 2021, p. 194), especially when working with the isolated self who never experienced a good enough environment. Karen Pando-Mars (2016), states that the AEDP therapist ́s offering of relational accompaniment to the insecurely attached patient can actually escalate the discomfort and increase the defensive strategies (p.23). The patient who has learned not to BE will require significant support to tolerate and take in the genuine connection with the therapist, gradually overcoming the painful reality of unmet needs, disavowed feelings, and unprocessed memories (Frederick, 2021). At times, we need to gently guide or actively help these patients toward connection.

Case vignette 2: Expanding receptive affective experience of being seen, intrapsychic focus (still in Phase 1: Triangle of Finding and Rescuing the Self ). A heightened affective moment.

In the next vignette we jump a bit forward in the process. I intuitively felt that Ellie needed to use me more, to feel my being with her, for her to be able to take the next step in her

development of self. I was “feeling the hole in her,” the lack of someone to lean on, the fear to BE and having her needs met. I use my therapist Self, self-disclose her impact on me.I tell her tenderly, while looking her in her eyes: ”I ́m here for you. When you are sad, I want to comfort you, when you manage to do something new, I feel delighted for your sake. You have an impact on me. Our interaction awakens feelings in me for you.”

Although this intervention is about helping her take in her impact on me and my deep care for her, it still needs to become an intrapsychic experience. She needs support to begin accepting and stop fearing the care and love she never received and stopped seeking during early childhood. This process is about increasing her receptive affective capacity and activating her attachment/seeking system -the beginning of “filling the holes.” It is through the felt sense of being “in the heart and mind of another” (Fosha, 2000, p. 57) that the (felt sense) begins to emerge. If you have never felt seen or cared for, you may struggle to feel you exist or care for yourself. And as McCullough (2003) states, “Nothing that isn’t received can be processed!” Ellie needs help to internalize and feel her own experience of having an impact on me.

This vignette illustrates my interventions to maintain the focus on her internal experience while helping her regulate the overwhelming emotions that arise at the edge of what her psyche can tolerate.

There is now a clear shift in the process. Ellie is beginning to have new experiences of presence within herself and with me in the room. In one session she expresses, “This is the first time I feel connected to myself AND to you at the same time.” She is safe enough to be vulnerable in a new way. It ́s the beginning of “feeling and dealing while staying related” (Fosha, 2000). Ellie experiences tremulous affectsfor the first time and realizes that ‘fear can be a good fear.’

Once I was equipped with Russell’s insight that “neglect, exploitation, and oppression of any kind leave holes of development of self” (Russell, 2021, p.244), I was able to understand the nature of the support Ellie needed from me in order to take a next, new and very important, step forward. My understanding helped to undo Ellie ́s shame for “not knowing how to do it.” Instead of believing that her feelings and behavior result from her own inadequacy stemming from defensive personality characteristics, she could now understand them “as a response to what, for most of our evolutionary history, would have meant actual death” (Sieff, D. F, 2019, p. 30). Most of all, that “the holes” are not who she is. This is the “essence of resilience,” as defined by Russell as the “self’s differentiation from that which is aversive to it.” (Russell, 2015)

Through my understanding, she began to recognize and accept that the absence of a space to BE during her childhood, and of what she had to sacrifice (her felt sense of being) was the only means of survival. It was at the level of ‘life or death, you or me.’ Later in the process, when Ellie finally entered the healing phase of her journey, she could see things with “new (adult) eyes” and shared a deep truth she had not been able, nor dared, to reveal before. She spoke about how her mother had used her as an extension of herself, and the mother’s disappointment and subtle punishments whenever Ellie expressed her own self/identity. We now understood how Ellie had learned not to BE, and how this had prevented her from developing her own self, instead creating these “holes.” My understanding represented another step in the process of undoing her aloneness.

Tremulous affects in AEDP refers to emotional states characterized by feelings of vulnerability, uncertainty, or shakiness that arise when a patient begins to experience positive emotional shifts or transformations. These affects often occur as the patient starts to connect with more adaptive emotions or begins to feel new, unfamiliar feelings of growth, healing, or connection (Fosha, 2000).

The next case vignette illustrates how the patient’s deepened connection with her own needs opens for a next step in the differentiation of her Self from her (internalized) mother. This is what is needed for Ellie to finally take her first step out into feeling she exists in her own right and that she can be and exist, have her own needs and wishes, without anybody else breaking.

PHASE 2: Triangle of the Emerging SelfCase vignette 3: The birthing of a whole and differentiated Self.

Ellie is now entering into the next phase between maladaptive and adaptive affective experiencing. She is developing and exploring the Emergent Self, and taking the first steps out of isolation, into a felt sense of existing.

The following vignette illustrates Ellie’s imitation of me as a first indicator of her emergence from a maladaptive into an adaptive affective experience. The imitative gestures might have been interpreted as a demonstration of a maladaptive state, but this behavior is now emerging as an expression of her unformed self. It is a precursor to the developing differentiation of Self through a deeper connection with her own needs and emotions.

The vignette illustrates the deep and moving breakthrough session in which Ellie, for the first time, experiences that she exists, has a felt sense of existing. She describes a physical experience beginning in her upper body and then spreading down through her whole body. It’s like a kind of birth, where I guide her through it – not just with verbal interventions but also by using my body and movements.

I wasn’t aware of my participation in this part of the nonverbal interaction until I reviewed the session afterward. It is then that I suddenly recognize a new phenomenon in the nonverbal interaction. Ellie is imitating my movements several times, in the way infants do. Meltzoff and Marshall, known for their research on infant imitation, argues that imitation in infants is not just a demonstration of memory but also a sign of the infant’s ability to understand and internalize the actions and intentions of others. It’s an adaptive function that enhances learning and survival, and it is used for social-communicative purposes. They state that through imitation, infants become like us. (Meltzoff & Marshall, 2018)

The emergence of the imitating phenomenon in the nonverbal communication with Ellie reinforces the idea that nonverbal communication is ”remarkably similar across the lifespan” (Beebe & Lachman, 2018). It also highlights the power of relationship and the innate drive to heal, grow, and develop aspects of Self that never had the opportunity to be nurtured when provided with the “right circumstances.”

The vignette demonstrates two main interventions. One is to support and help Ellie stay with, deepen and expand the emergent felt sense of her needs and will. Ellie experiences for the

first time a transformational spiral, i.e. the experientially processing the experience of transformation, giving rise to yet another round of transformational affects (Fosha, 2021). The stretch and deepening of the felt sense of her needs and will, along with the right to them, deepens into core state experience of “This is who I am. I take responsibility for my Self, and this is who I want to be.” As this experience of Self lands sufficiently enough there is a sudden opening in the process of becoming separate in the relationship with her mother. She can for the first time feel two feelings at the same time. In next step she can be angry at her mother without the fear that her mother will fall apart. From this new space of being separate from her mother she is now open to take her first steps into the felt sense of existing.

My second intervention is to emotionally, verbally and nonverbally guide and support Ellie ́s becoming/birthing while taking her first steps out of emotional isolation into existing by using my therapist Self and my body to support. WE are now level by level filling “the holes” in the development of her Self.

This vignette illustrates how Ellie, now feeling safe enough in her emerging Self, can release her early survival strategy of protecting her mother. Her emergent Self is established enough for her to face the truth and see both herself and her mother as separate individuals. Letting go of the early insecure attachment bond to her mother, and the need to prioritize her mother’s needs, frees her from the necessity to sacrifice her own needs, her Self, and her existence. This allows her to enter a new space where her unique Self can begin to fully exist, and this newfound freedom activates the early adaptive function of imitation, one of the most important interactive features for learning and survival (Meltzoff, 2017).

In this article I explore and illustrate the use of a fine-tuned AEDP stance, and the benefit of following the phenomenology in work with emotionally isolated individuals. Commitment to the AEDP stance and to tracking the phenomenology through AEDP’s moment-to-moment processing has led to my insights and opened new areas of work in State 2 and in between maladaptive and adaptive State 2 phenomenon. The transcripts demonstrate that bottom-up processing, i.e., nonverbal, right brain-to-right brain, moment-to-moment experience, (balanced with left-brain understanding), promoted Ellie’s agency and built her relational capacity to trust in the therapeutic process and in our innate capacities to BE, as well as to be together. Additionally, it is a demonstration of Fosha’s assertion that this approach fosters motivation for both therapist and patient to explore and learn. She states, “Phenomena keep us humble and always learning” (Fosha, 2021, p. 31).

By closely tracking and staying grounded in moment-to-moment experience, I was able to learn, experientially, what Ellie was asking for and needing from me, and what interactions she best responded to, especially during when she couldn’t convey her experience in words. AEDP’s foundation in infant research helped me to identify when she felt met by me, and to identify markers of growth. For example, I recognized that her imitation of me was a form of communication rooted in our evolving secure therapeutic attachment, indicating she was in the earliest stages of developing her sense of Self. In fact, all the phenomena that occurred when Ellie was given intersubjective space informed my understanding of the work, and what needed to be identified, felt, and developed.

The new triangles I presented in this article, The Triangle Of Finding And Rescuing The Self and The Triangle Of The Emerging Self, represent an overall formulation of how to find, form and transform the unformed Self within the AEDP framework. It is not until a felt sense of existing, of being as one’s True Self, is sufficiently established that the healing and processing journey can begin. It ́s an experiential journey in depth, and in many, many steps. Like one of my patients so beautifully explained, “Everything contributed to this (the birth of my Self). Every single word, every feeling…every single tear.”

When I asked Ellie, now finally in the adaptive affective realm of experience, what it felt like to feel whole, she answered, “It feels like I have become a person.”

Agency is resilience in action: The role of recognition in transforming resistant agency to resilient agency

Abstract: The concept of agency has recently been introduced into the conversation about attachment-based psychotherapy. Agency can be a manifestation of resilience, the intrinsic capacity of all living organisms to differentiate between what is beneficial and what is harmful. Although human agency is often equated with the expression of autonomous action, it also functions in the background to provide the relational safety required for autonomous functioning through receptivity and mutual recognition. Relational safety begins with attachment and evolves into varied forms of affiliation that provide security and entail the obligations and accountability that constrain autonomy. The resilient “self-at-best” might be characterized as a state of optimal balance between autonomy and accountability. Agency can also serve a “resistant” rather than resilient function for individuals who do not experience sufficient relational safety, operating in the service of a “false self” or the “self-at-worst.” Psychotherapeutic interventions can restore an adaptive balance between autonomy and relational safety. The relational safety that attachment-based treatment provides is necessary, but it is not a sufficient for healing when a client’s agency is compromised – when agency is resistant, inhibited or misdirected, and not resilient. “Attuned disruption” of resistant agency may be necessary to restore and support the resilient function of agency and promote transformance. Case vignettes illustrate the resistant use of agency in misdirecting desire, disguising capability, and sacrificing growth to preserve a sense of relational safety. Active and receptive modes of agency are considered with regard to the importance of mutual recognition in resilience and transformance.

Introduction and overview

We want to fit in, and we want to stand out. We want to feel the safety of connection to others and at the same time the thrill of autonomy. As Shunryu Suzuki, Buddhist master and master of the Zen one-liner, observed, “If it doesn’t sound like a paradox it probably isn’t true.” While attachment theory has displaced psychoanalytic drive theory as a more useful model of human motivation, attachment is only one aspect of what motivates us. Relational safety is a necessary but often not sufficient condition for emotional healing and growth. Human existence requires maintaining a balance between the need for autonomy and the need for relatedness in order to assert our independence and also enjoy the security of connection to others. The term “agency” is often employed to refer to actions that are assertions of independence, particularly in contexts where human interdependence is insufficiently acknowledged and appreciated. Human development has often been mischaracterized as a linear process of increasing autonomy, not the ongoing interplay of individuality and affiliation. But perhaps the “relational turn” in psychodynamic thinking, and the emphasis on relational safety, of which attachment theory is a prime example, was an overcorrection.

Eileen Russell (2021), in her important and thought-provoking article, introduced the concept of agency and the term “agentic self,” into our attachment-centric conversation. Russell’s agentic self is the “embodied sense of being able to act in the world, to have an impact on one’s surroundings and important relationships, and the felt sense of connection between one’s will and desires and one’s goals” (Russell, 2021 p. 246). In an earlier contribution to the AEDP canon, Russell emphasized the role of resilience, defining it as “the self’s differentiation from that which is aversive to it” (2015, p.5). Agency translates desire into action, wanting into willing. If resilience is the capacity to realize one’s desires, then agency is resilience in action.

In the broadest sense, agency is purposeful action, a property of all living organisms. The exercise of agency entails an awareness of need or desire, the capability to take action to fulfill that need or desire, and the evaluation – whether implicit or explicit – of the safety of that action. While agency is often equated with action and assertions of autonomy, in social animals, including humans, it also serves to ensure compliance with social norms, thus serving to maintain relational cohesion in the group. Compliance with social norms is motivated by the desire for connection, not autonomy, and is expressed and recognized as an affiliative intention. Because other animals have co-evolved with their ecological niches, they are “naturally” resilient, insofar as they are epigenetically equipped with the capabilities required for survival and reproduction. Transformance, the upper limit of resilience, is, with few exceptions, largely limited in other animals to the expression of those genetically determined capabilities, with change occurring primarily through differential reproduction rates over generations.

Human agency is uniquely complicated because human life requires both autonomous functioning and the maintenance of relational security within a complex network of social groups and hierarchies. Human infants are equipped with less developed and more plastic nervous systems and are raised in a wide variety of physical and social environments. Different cultures, as well as different families and other groups within the same culture, have varying levels of tolerance for violating social norms. The exercise of agency in balancing the needs for relational safety and autonomous action entails the consideration of family, social and cultural norms, and of needs that emerge throughout an extended and complex life cycle. As a consequence, human existence offers, and requires opportunities for internally motivated transformation, individual expression, and innovation that other animals do not enjoy. It allows for a wider variety of ways to express and experience both autonomy and connection but carries the risks of existential isolation that other animals are largely spared.

At our best, we enjoy a sense of competence in our exercise of agency, and a sense of connection from recognizing others and knowing that we are recognized in return. Our needs for autonomy and for connection are in an adaptive dynamic balance, and we experience a sense of integration and coherence. If our exercise of agency is the translation of desire into action, it requires confidence in our capabilities and an evaluation of the consequences of that action. At our best we can recognize and tolerate the emotional arousal of desire, realistically evaluate whether we could act on our desire, and whether we should. Resilience is knowing what is best for us, the maintenance of an adaptive balance between autonomy and connection. However, we are not, and by definition can’t be, always at our best. Shame and guilt may interfere with our ability to access the embodied expressions of desire or make the risk of failure feel too painful. We may be too afraid of risking the disapproval of others to whom we feel accountable, whether that disapproval is real or perceived, and, fearful of abandonment, we may abandon ourselves.

Realizing new desires, risking failure by extending our capabilities, and re-evaluating our accountability to ourselves and to others over the course of youth, adulthood, and senescence, are manifestations of

transformance, the highest form of resilience. Transformance is bi-directional – both reparative and expansive, – and it has both active and receptive aspects. Like the roots of a plant, it seeks nurturance, and like the leaves, it grows upward toward the light.

Part I
Agency and living systems: Biological and cultural considerations

Agency in the broadest sense is the goal-directed and purposeful action that is a
defining characteristic of all living systems (Ball, 2002; LeDoux, 202 3; Thompson, 2007). Purpose does not necessarily imply conscious intention or a sense of identity, but simply the capacity to survive and reproduce, to differentiate between what is nourishing, neutral, and aversive. Living organisms maintain an internal metabolic equilibrium by maintaining a boundary between themselves and their environment, incorporating what is nourishing and excluding or eliminating what is toxic. If resilience is “the self’s differentiation from that which is aversive to it” (Russell, 2015), then perhaps it is an essential characteristic of all living organisms, even those without an explicit concept of selfhood. Biologists, some biologists anyway, attribute “minimal cognition” to all living organisms, since their survival requires an evaluative capacity (Godfrey- Smith, 2016). Daniel Dennett refers to this as “competence without comprehension” (2017, p. 336). Biologist J. Scott Turner (2017, p. xv), venturing into “the modern metaphysics of biology… squishy things like purposefulness, intentionality, and consciousness,” argues that agency is “the ineffable striving of all living things to become something.” 1 Gendlin (2012), reflects if Aristotle had had the opportunity to contribute to the discussion, he would have recognized agency as “anima” – the energy that drives plants to seek nutrients, animals to locomote through their domains, and humans (at least on some occasions) to reason.

Agency in animals involves the evaluative exploration of the environment under conditions of safety to seek nourishment or opportunities to reproduce. Animals distinguish between opportunities to have lunch or be lunch, and in the latter case whether to fight, flee, or freeze. The life cycle of most animals follows a simple trajectory of a relatively brief juvenile phase, adulthood with reproduction, and decline. Social animals for the most part remain in a stable group for the duration of their adult lives. The agency of animals – their needs and capabilities – is biologically synchronized and matched to the affordances of the environmental niches they occupy as a consequence of evolutionary adaptation.

It might be argued that animals, if not all living organisms, with the exception of human beings, are naturally resilient, as long as they continue to reside in the ecological niches to which they are genetically adapted. If biological and psychological development is understood as epigenetic, then a stable environmental niche provides the necessary conditions for the expression of genes associated with traits that have evolved within that niche – or perhaps co-evolved with – that niche. Placed in environments sufficiently different from their niches, such as traditional zoos, animals often manifest behaviors that might be termed symptomatic of the emotional disturbances seen in humans (Morgan and Tromberg, 2007). Because we are not epigenetically

fully at home.

Humans, of necessity, have developed the capacity to adapt to increasingly complex physical and social changes. Human existence takes place in a variety of physical milieus increasingly modified or constructed by perpetual human innovation, what philosopher John Gray (2007) terms a “prosthetic environment.” Along with a constantly evolving material world of functional artifacts, we exist in a social environment of multiple groups and social hierarchies, with different norms and practices, necessitating constant code-switching. The human life cycle barely gets started when we have reached adulthood.

So, if resilience is understood as the capacity to maintain an internal sense of coherence and an adaptive balance among desire, capability, and relational safety, then human existence requires more complex manifestations of resilience than those exhibited by other animals. Human beings are born without the innate potential competencies to survive in an environment to which we are epigenetically adapted. Humans are, however, innately endowed with a disposition to respond to others, and to elicit nurturing behavior from parents. We are born into a wide variety of sociocultural environments in which we must learn to survive and thrive. We also need to keep changing, necessitated by our growing older after we have grown up, as our bodies, our needs and desires, our agentic capabilities, our opportunities and obligations change within a social environment that is also constantly changing. If you grew up with manual typewriters and dial telephones, please raise your hand.

Philosopher Charles Taylor (1985, p.16) argues that humans are “self-interpreting
animals” whose exercise of agency involves a “strong evaluation,” not only considering the practical consequences of action but also how action would be received by others. Taylor’s philosophical approach reflects a relational turn, characterizing selfhood as a “dialogical” process, and highlighting our inherent accountability to others. Both Taylor, whose focus is on human selfhood, and Kim Sterelny (2001), who examines the evolution of agency from a biological perspective, use the term “second order” to refer to this reflexive, and implicitly relational aspect of human cognition.

Humans are unique in possession of the capacity not only to develop the agentic
capabilities required by their environments, but also to imagine new possibilities, and to consider not only what we can do, but what we could do, and what we should do. Animals demonstrate many insufficiently appreciated cognitive capacities and can recognize novel uses for things in their environments. Primates and corvids (crows and ravens) use available objects as tools, but do not create tools for future use2. Humans do

not merely adapt to their environments, they adapt their environments to themselves, or create new environments, beyond making nests. This is not always something to be celebrated. Philosopher Thomas M. Alexander (2013, p. xxxii) suggests that “the main problem of modernity is that the power of science has given us has not been matched by the wisdom to use it.”

If resilience and transformance are, echoing Turner (2017), expressions of “the ineffable striving to become something,” then the “something” that other animals strive to become is the best version of their species. Their agency, which includes their capacity for evaluation, is substantially determined by genetics.

Human agency is a product of biological plasticity and cultural adaptation. The human capacity for “strong” and “second-order” evaluation does not emerge in a uniform way. It follows different paths in different cultures and requires continual revision over the life span. Formulating “second-order” evaluations require more than the resilience exhibited by other animals, despite the similarities. The same actions that are socially acceptable in some contexts may not be acceptable in others. Social practices that marginalize some groups have been recognized as unjust and are challenged by some and defended by others. As we grow, the revision of our “second-order” evaluations – whether spontaneous or assisted by a psychotherapist, requires a “third- order” evaluation, or meta-evaluation, of the “second-order” evaluations that we have been using. In the modern world, embracing and then questioning the evaluative norms instilled by our parents is normative. Pairing and parenting entails reconciling the second-order evaluations of each partner to achieve, with varying degrees of success, a consensus regarding the principles, policies, and practices that regulate family relationships.

No wonder why we feel like we are always playing catch-up. If you grew up with manual typewriters and dial telephones, you can put your hand down now.

Part II Human Agency: Developmental and clinical considerations

Now the first thing I learned in Accelerated Experiential Dynamic Psychotherapy, ironically enough, was to slow things down, and in that spirit, I want to spend a little time parsing the concept of agency.

Agency is often equated exclusively with autonomy. For example, Safran and Muran (2000) in their consideration of fifteen “Fundamental Assumptions and Principles” in psychotherapy, begin by addressing “agency versus relatedness…the innate needs both for establishing and maintaining relatedness to others and for self-definition or individuation” (Safran & Muran, 2000, p.31). They cite David Bakan’s The Duality of Human Existence: Agency and Communion (1966) as a primary source of this notion of dual motivation and argue that “the resolution of the tension between the need for agency and the need for relatedness is a dialectic

that lies at the heart of psychoanalytic theory” (p.31). This semantic ambiguity obscures the foundational role of agency in providing relational safety as the context in which autonomy is both exercised and constrained. The conflation of agency and autonomy is an instance of the cultural bias about which Russell (2021) comments: “I am aware of the potential limitations of these developmental theories across cultures, particularly ones that do not prize individuality. For now, I am happy to limit my discussion to Western cultures in which the self is arguably too prominent” (p. 245). Philosopher Edward Harcourt (2018) has observed that psychoanalytic terminology includes invidious distinctions such as immature and mature, or merged and separate, which refer to psychological maturity, mental health, and by implication, moral virtue. He notes that even relational psychoanalyst Stephen Mitchell includes “fusion” and “autonomy” to his list of naughty and nice. The very notion of autonomy presupposes that any particular “autonomous” action is a contrasting figure against a ground of norms that are observed. If agency is resilience in action, then relational action to support connection, and the inhibition of actions that threaten social cohesion are also expressions of resilience.

The early development of the agentic self:

The initial function of agency is to both facilitate attachment – and the relational safety it provides – and to provide experiences for the earliest manifestations of selfhood (Benjamin, 2018; see also Stern, 1985/2000). The human infant develops a sense of agency and a sense of relationship to others simultaneously. If agency is action that has a purpose, if not necessarily intention, then the first manifestation of agency is the newborn’s cry of distress, which elicits a comforting response from the parent.

Jessica Benjamin (2018) has described the developmental trajectory of agency, from initial total dependence to the robust sense (or illusion) of autonomy, provided by implicit relational safety. Agency is built by the parent’s provision of various forms of recognition “in different registers.” It emerges from moments of “rhythmic” attunement, of “meeting” – a terminology Benjamin attributes to Louis Sander. “Marking” constitutes recognition in a different, meta-communicative register, in which “the mother simultaneously exaggerates and mirrors the baby’s reaction to show that she understands the baby’s distress but is herself not upset” (Benjamin, 2018, p.149). This process involves “a mixture of rhythmic and differentiating aspects (p. 91). Recognition in this context does not simply imply the registration of something familiar, but the realization of something new, vital, and true. The affirmation of agency by the parent is the process that quickens the sense of agency while providing relational security. Parenting an infant is, among other things, a sequence of attunements and misattunements, ruptures and repairs. “A vital outcome of interactive repair is the infant’s own emergent sense of agency and impact on the mother. Her response to her child’s impact in turn confirms the experience of mutuality” (Benjamin, 2018, p. 88).

Mutuality for Benjamin requires “surrender,” the acknowledgement of the agency of the other without relinquishing one’s own autonomy, neither submission to the agency of the other nor assertion of one’s agency to dominate the other. Benjamin allows that this mutuality is “an ideal state of relatedness [that] can’t be sustained” (p. 7) – like the “self- at-best,” insofar as we can’t, by definition, always be at our best. Mutuality is difficult to sustain, and we all inevitably, to different degrees, collapse into the complementarity of “domination” and “submission,” “doer” and “done-to.” Development can be characterized by increasingly differentiated and integrated functioning (Werner, 1940), by a growing capacity to balance independence with intimacy, and by both confidence in our capacity to will our desires into action and a willingness to surrender to the mutuality that Benjamin describes.

Attachment, for better or worse, establishes a relationship to the world of others – a world that may offer comfort or withhold it, that can be consistent or unpredictable, safe and welcoming, chilly and disapproving, or simply indifferent. The repair of ruptures has both an active and receptive aspect for both parties, a mutual recognition and surrendering. Emotional difficulties begin when we are forced to choose between relational safety and autonomy, or when misattunements and ruptures are not repaired. “As an adaptation-based psychotherapy…AEDP assumes that psychopathology reflects a person’s best efforts at adapting to an environment that was a poor match for the person’s emotional and/or self-expression. Thus, even the most self- destructive or disturbed presentations can be seen as manifesting hope, self- preservation and ingenuity” (Fosha, 2008).

If autonomous strivings jeopardize relational safety early in life, a defensive and
inhibitory mode of agency would initially be an expression of resilience. But later in life “adaptations carried over from an earlier time…may no longer be helpful” (Russell, 2015, p.57). Without the development of sufficient “receptive affective capacity” (Fosha, 2009), expressions of autonomous agency remain inhibited, and interpersonal functioning would be skewed toward maladaptive compliance or avoidance. In the absence of relational safety, receptivity is experienced as submission rather than surrender, receptive affective capacity cannot grow, and resilient receptivity is inhibited. The legacy of attachment failures restricts our ability to sustain a necessary degree of mutuality – that sense of oneself as having agency while simultaneously acknowledging the agency of the other.

Some of us relinquish our resilient desires and fail to recognize our capabilities, striving instead for an unattainable degree of relational security and anxiously seeking excessive reassurance, permission, or approval, or by complying with a sense of defeat, resentment, and powerlessness. Some of us protect our sense of autonomous agency by avoiding connection with others.

Agency can operate in a resistant mode as well as a resilient one. Without sufficient relational safety, the “self” on whose behalf agency is exercised, is a compromised self, a “self-at-worst,” a “false self” that relinquishes growth for a sense of security. Resilient agency can become resistant agency. Both the active and receptive aspects of resilient agency are inhibited. While agency is resilience in action, agency is not always an expression of resilience, and resilience is not always manifested in action.

Emmanuel Ghent, one of the godfathers of relational psychoanalysis, has observed our conflicting basic desires – on the one hand, the need for safety, stability, and coherence; on the other, a drive for “new experiences that result in growth and expansion of the self.” He adds that all human activity takes place within this “human envelope” (2018, p.63). Growth of the self, transformance, and the expansive exercise of agency, requires sufficient relational security – that “human envelope.” We live in what Alasdair MacIntyre describes as “communities of giving and receiving.” He argues that we are “dependent rational animals” whose lives begin, and often end, in states of helplessness, and include inevitable episodes of illness or injury (MacIntyre, 1999).

Relational safety is both the precondition of and the platform for expansive and
exploratory action. If we are scanning the environment for threats, we will neither be seeking satisfaction nor exercising curiosity. Our primary desire is for safety, and openness to change and curiosity are possible only under conditions of safety. Russell (2021, p.260), citing Jaak Panksepp’s research, refers to “safety promoting exploration.” The exploration of the world is the figural activity against a ground of confidence, familiarity and

predictability, derived from secure attachment. The “practicing” phase of development described by Margaret Mahler and her colleagues (Mahler et al., 1975) exemplifies perhaps the earliest manifestation of what Russell (2021) terms an intentional and autonomous “agentic self,” whose tacit awareness of the mother’s presence in the background allows for attention to be directed to exploring the environment as an individual. Agency then begins to be exercised in the service of separation and individuation as well as connection.

If the implicit sense of relational safety in the background makes autonomous activity possible, in a sense maybe we are always just practicing.

Agency: From attachment to affiliation and accountability

As we grow, our desires change, our capabilities increase, and relational safety is
provided by interpersonal connections that evolve from our attachment to our parents and to complex networks of relationships and affiliation. Our identity as autonomous selves is derived from our being recognized by others, an irony Jessica Benjamin has observed, and Eileen Russell cites, “The need for recognition entails this fundamental paradox: in the very moment of realizing our own independent will (agentic self) we are dependent on the other to recognize it” (2009, p.39). Different forms of relatedness are provided by siblings, friends, classmates, teachers, teammates, partners, colleagues, children, coreligionists, political allies, and fellow fans of athletic teams and popular artists and entertainers (have I left anything out? I’m certain I have. Feel free to add to the list in the margins). Affiliation provides security and entails obligations to others. Accountability is the price we pay for belonging.

Just as the individuals to whom we are connected change, the forms of accountability we experience change. We owe parents a diminishing level of accountability as we grow and enjoy greater independence and autonomy. Accountability is what we offer in exchange for an allowance, and later for the car keys. Fairness is the entry fee for participating in peer groups of equals, although some peers are invariably more equal than others, and some groups not as egalitarian as others, and a degree of submission may be mandatory. A more abstract form of accountability emerges when along with significant others we hold ourselves accountable to a “generalized other” (Mead, 1934).

We learn “not only how to speak, but also how to speak for the other” (MacIntyre, 1999, p.150). We are not only responsible to others, but for others. Accountability includes accepting legitimate authority (stopping at red lights), exercising benevolent authority (such as with one’s children, at least until they reach the age of 30 or 35), as well as honoring intrarelational commitments to one’s own values and priorities. Accountability may also require challenging illegitimate authority, when one’s own rights, or the rights of others, are being violated.

Resistant agency and therapeutic considerations

Eileen Russell has introduced the concept of agency into our conversation regarding attachment and how to help people achieve a sense of coherent, confident, and connected selfhood. The inclusion of resilient agency as a complement to relational safety, enriches our working vocabulary, making the emphasis on resilient agency within the context of attachment more explicit and precise. Additionally, she expands State 2

maladaptive core affective experience to include the condition of unformed agency,3 and introduces the concept of defensive agency to State 2 maladaptive experience. Russell has integrated into AEDP the notion that agency can be recruited in the service of defenses by operating on behalf of the self-at-worst. She identifies this phenomenon as “resistant agency.” Resistant agency is active, though resistant rather than resilient.

Resistant agency can emerge as a significant issue in psychotherapeutic treatment, requiring unique interventions. She likens the inhibiting effects of resistant agency to Otto Rank’s notion of “negative will,” a form of paralyzing receptivity. She describes patients who, despite enjoying safety in the therapeutic relationship are nevertheless inhibited by maladaptive shame and helplessness engendered by negative will. She advocates “pressuring with empathy,” to confront the patients’ implicit willingness to abandon themselves. Now she has upped the ante, arguing for the necessity of the therapist playing the role of a “transformational other,” providing not only relational safety but also “attuned disruption.”

Relational psychoanalyst Karen Maroda has recently voiced a similar concern, noting that “Research on early attachments and the need for holding and empathy have prevailed as a model for clinical interventions, creating a parentified and overly passive approach to doing treatment (2021, p. 114).” Don’t confuse relational safety with comfort, Russell and Maroda advise us. Relational safety is a means, not an end. Not a home, but a foundation on which to build a home. As Fritz Perls observed, psychotherapy should be a “safe emergency,” if something new is going to emerge (Perls et al. 1951).

In the absence of sufficient relational safety, agency may be misdirected, serving what Leigh McCullough Vaillant (1997) termed a “hidden agenda.” A history of misattunements and unrepaired ruptures can result in the recruitment of agency in the service of preserving a sense of safety at the expense of growth. If the development of agency is hijacked by a lack of relational safety, agency may be used to seek approval and permission from others, or attempt to control or avoid others, rather than satisfy autonomous desires, at the cost of a more authentic connection with others, and with oneself. The result is a compromised and “false self” that is to varying degrees deficient in the awareness and achievability of desire and is more responsive to others rather than to one’s own core experience.

This resistant use of agency needs to be identified and disrupted in the context of a
therapeutic relationship that provides sufficient relational safety. As Emmanuel Ghent (2018, p.54) wisely suggested, “There is more than one way to help a flower grow. Sometimes selectively nourishing or watering is what is required, sometimes clearing the weeds.”

In practicing Accelerated Experiential Dynamic Psychotherapy, we have been implicitly promoting our clients’ agency all along, of course, as well as providing relational safety. We ask permission before probing sensitive issues, and metaprocess our interactions with clients. We acknowledge and repair ruptures in the therapeutic relationship, as do other relational psychodynamic therapists when they address “enactments.” But we also privilege the positive, explicitly expressing our joy and pride at the growth we observe in our patients. We recognize the joint contribution to misattunements, but also celebrate the co-creation of new understandings of painful past experiences. We recognize the victory of courage over fear, in both initiating new autonomous action and in greater relatedness and receptive affective capacity.

What follows is a consideration of the nature of agency, both resilient and resistant, in both its active and receptive modes, and its relationship to transformance. Case vignettes will demonstrate the utility of an agency-based perspective in psychotherapy, as a complement to other perspectives.

PART III

Clinical examples: Agency misdirected, exercised but unrecognized, or overdetermined

The exercise of resilient agency requires awareness of desire, a sense of capability,
and an evaluation of accountability to ensure that action will not jeopardize relational as well as physical safety. The functions of resistant agency are to suppress / inhibit adaptive or transformative desires, deny capabilities, and adhere to a sense of accountability that affords a sense of safety at the expense of flourishing. To address problems of agency with attuned disruption, attunement must precede disruption. Having established a relationship with relational safety, we can activate potential resilience and receptive affective capacity. Following Russell’s advice and looking for what is missing, we might recognize desires obscured by “hidden agendas,” identify previously-unrecognized capabilities, and address accountabilities that are the legacy of misattunements and unrepaired ruptures.

The following treatment episodes demonstrate how agency has been employed to preserve safety at the cost of growth and how disrupting resistant agency makes room for resilience. These examples reveal agency that is misdirected, exercised yet unrecognized, or overdetermined, serving both resilient and resistant desires. Nevertheless, the very act of seeking treatment reflects a glimmer of transformance striving and what Russell describes as potential resilience – a capacity that coexists with resistant agency.

In the following three case vignettes, identifying information has been omitted or altered.

Vignette 1:
Bernard: Agency misdirected:

Bernard initiated therapy prompted by his inability to finalize the purchase of a motorcycle he wanted and could easily afford, having on several occasions placing a deposit on the motorcycle but then cancelling the transaction. The responsibility of even a modest financial commitment prompted acute anxiety. While Bernard remained close to a group of longtime friends, he took few initiatives to see them, waiting for their regular invitations, and made no efforts to establish new relationships. He had had one extended romantic relationship which ended a number of years before because he was unwilling to commit to marriage and children.

Bernard lived in a house he had inherited a decade before from a childless uncle and aunt. Although he did not see himself as particularly handy – unlike his father and younger brother, he devoted himself to making repairs and improvements. His installation of new baseboard in the living room, and the challenges it imposed were a major focus of the initial sessions. Bernard was deeply committed to doing an acceptable job, and seemed to accept the fact that he did not have the skills that his father and brother had. But the therapist realized that something was missing, that the therapist was missing something. The therapist pivoted from focus on

to the nature of the desire itself, and Bernard’s “hidden agenda” (McCollough Vaillant, 1997).

Bernard’s relationship to his father emerged as a primary theme in treatment. Bernard reported resentfully “holding the flashlight” for his father while the latter was involved in various projects or having to assist his father in meticulous groundskeeping. Bernard just wanted to play with his friends, or have his father play with him. But Bernard’s father was nevertheless a source of reassurance for him, tacitly approving of Bernard’s choice to pursue an advanced degree in an area of which his father had little understanding. Bernard yearned for his reserved and slightly formal father to be expressive and physically affectionate, and he saw his father as a sad and martyred figure, enslaved by responsibilities to the house and grounds, Bernard’s mother’s urgent demands, and by his father’s devotion to the Catholic church. Bernard’s father often left the house early to assist in the service of an early morning mass before going to work. His father seemed to have no desires of his own, and Bernard had fantasies of buying his father a sports car, although he knew his frugal father would have been perplexed rather than delighted by the gift and concerned about such a frivolous and impractical purchase.

In the spirit of “attuned disruption,” the therapist began to question the accuracy of
Bernard’s characterization of his father, whose life as described sounded to the
therapist purposeful and satisfying if not joyous. The therapist noted that Bernard was quite different from his father in some ways and like him in other ways. Both Bernard and his father were committed to being of service to others, although his father’s service was grounded in his deeply held religious beliefs, while Bernard chose a secular route to provide service to others. The therapist observed that Bernard’s father’s commitment to being of service to others was not “servitude.” Bernard became aware of how much of his commitment to home maintenance and improvement, about which he frequently complained, had to do with his wish to have been closer to his father, but did not achieve that implicit goal, and he began to reflect on the extent to which this conflicted with acting on his own desires.

Patient: Everything comes back to him. Why I couldn’t make him happy. I always wanted to make him happy. I always assumed there was something to fix.

Therapist: As opposed to accepting him for who he was. Being loyal to him to try to feel closer to him, but not being true to yourself.

Patient: It still hurts sometimes. I just wish I had experienced more closeness.

In subsequent sessions, Bernard began reflecting on how his father was different from him, and considering the possibility that while he imagined his father was sad, he could have been actually content doing something that Bernard himself would have hated doing. He recognized the differences and the distance between his father and himself, and the sadness that came with that realization.

At the next session, Bernard began by reporting that he had visited his father’s grave after the previous session, “closing something.”

Patient: I understand who he was, how he was. I wish he’d followed in my footsteps a bit. But he couldn’t. I had to express that to him…Coincidentally, my father’s grave and the bike store are both in (name of town). I

looked at the bike. Then driving home, I got anxious…I hate maintaining the house. I can’t do it like Dad would do it. I was always prioritizing the house, me versus the house. Always either / or, never a balance…. I’m struggling accepting that my Dad liked to work.

Therapist: You like to work. Just not the same kind of work…

Patient: (recalled eating liverwurst sandwiches with his father, and both enjoying them.) It was a true moment, he and I were the same…I miss that…Something was just coming up, but it gets stuck here (indicates his chest). That was my Dad being affectionate, bringing me along with something that feels good. I hadn’t labeled it as his way of being affectionate. It feels very settled, peaceful. This feels like the important stuff I got. The other stuff feels like bullshit, not as relevant.

Therapist: It was real, not what you imagined

Patient: Oof…It’s a different level. My experience of my Dad, not a cognitive thing.

Therapist: Thoughts in your head, as opposed to something actually between you.

Patient: Thanks for that. It puts me in a space I haven’t been…

Vignette 2:
Wendy: Agency exercised but unrecognized

Wendy had been employed at a geriatric facility for almost a decade. Over the course of her employment she demonstrated unique interpersonal skills leading to a significant promotion. In recognition of the crucial role she had been playing, a new position was created for her, with a substantial increase in her salary. Prior to her promotion, she had initiated psychotherapeutic treatment to deal with anxiety concerning her competence at work, and despite her promotion, she continued to doubt her interpersonal skills. She attributed her success to “just being a hard worker.”

While Wendy was quite evidently sensitive, confident, and decisive in dealing with the delicate politics of the facility, and was highly regarded, she was often anxious and self-critical in her accounts of her work activities with the therapist. She would interrupt herself with self-deprecating or apologetic remarks, and provide excessive, and unnecessary, amounts of detail to justify decisions she made and actions she initiated. Although she was clearly extremely effective, she was unable to recognize the agency she was exercising. While recounting what she had done, her emotional state shifted, and she became anxious and self-critical, despite the appreciation of her “clients” and the acknowledgement by the facility’s administration and coworkers.

Wendy’s older sister was a university professor, and in the autobiographical narrative that emerged over the course of therapy, her sister was “the smart one” and Wendy was “the social one.” Wendy’s sister was an academically outstanding but extremely shy individual, and their mother at one point gave her a book entitled “How To Make Friends.” If Wendy’s sister was smart but not social, Wendy concluded, then her being social implied intellectual deficiency. She revealed that her dream was to become a psychotherapist, but she believed that she, unlike her sister, was not smart enough to achieve that goal. “I’m just a hard worker.”

The therapist contrasted Wendy’s beliefs about herself with her accounts of her
spontaneity, sensitivity, competence and confidence. She became aware that her image of her sister was a frequent presence when she was dealing with peers and those she regarded as authority figures, including the therapist, and became aware of shifts from confidence to self-consciousness in her interactions. Her receptive affective capacity developed to the point that she was able to accept another advancement in her career, and her effectiveness was recognized by her supervisors, the administration, and increasingly by herself.

Frank: Overdetermined agency, resilience with a “hidden agenda

Frank and his wife were referred for couples therapy by the wife’s psychotherapist, but within a few sessions it was clear that Frank’s absorption with work made him emotionally, and even often physically unavailable to participate in family life. When his wife’s distress was made more explicit and urgent to Frank, he agreed to start individual sessions. By his mid-forties, Frank had ascended to a position as executive officer of a mid- sized corporation. He was gifted with considerable organizational and managerial skills as well as with warmth, sensitivity, creativity, and a sense of humor. In one of the smaller companies, he had worked for on his way to the executive suite, in addition to his varied responsibilities in operational and fiscal areas, he became the de facto Human Relations director – the member of the management team best equipped to deal with employees on a personal basis.

Frank was deeply committed to his children and his wife – who like him was highly intelligent, ambitious, multi-talented – in principle, but in practice he was so deeply absorbed in his work that his availability, even when he was home, was constrained by telephone calls and emails to which he felt compelled to reply. He derived tremendous satisfaction and a sense of pride in his meeting increasing professional challenges and providing generously for his family, and his presentation reflected enthusiasm, risk tolerance, and vitality. Perhaps not coincidentally, on his first date with his wife, when they were barely out of high school they went sky diving.

On the few occasions when his wife became angry with the work-life imbalance that Frank maintained, he became briefly but profoundly depressed and felt a deep sense of abandonment. As therapy proceeded, Frank explained that along with the exuberance he experienced in finding new challenges at work, and even at home (he was an extremely gifted chef, and often enlisted his children, who enthusiastically participated in meal preparation), he felt that he had to “earn his keep.”

Frank, the oldest of three children, was charged with preparing school lunches for himself and his younger sisters from almost as early as he could remember. By the time he was eight years old, was tasked with preparing dinner and supervising homework. His father was a colorful figure who worked in popular media and travelled a great deal of the time, and his mother was involved in varied activities that kept her away from the home many evenings. In reporting this, Frank was rather matter of fact, and it appeared that he accepted those unusual responsibilities, along with conventional chores such as making his bed in the morning, as normal and reasonable. Challenges, even ones that might have been burdensome and onerous to others, seemed to be opportunities to exercise his capacities for problem solving and leadership. There appeared to be minimal conflict among Frank and his sisters, who willingly assisted him in preparation and clean up, and when he left home for college, one said her life had been ruined by his departure.

Frank’s “earning his keep” was a “hidden agenda” (McCullough Vaillant, 1997) that was obscured most of the time by his exuberant exercise of resilient agency, but while he was extraordinarily good at doing, he did not much capacity for just being. The occasions when his wife expressed her dissatisfaction and withdrew from him had caused Frank tremendous distress. He became aware of the uneasiness he felt if he was not constantly performing, or in one way or another, “earning his keep.” Frank’s parents never acknowledged that his fulfillment of his extraordinary family obligations was anything other than routine and completely taken for granted. Over the course of therapy he began to realize that a component of fear had been obscured by the joy he derived from meeting challenges. He and his wife began to make time to be together without any other agenda, and Frank’s life began to assume a healthier and more satisfying balance.

Conclusions from clinical cases

The exercise of agency involves the awareness of desire and the evaluation of one’s capability and accountability. The vignettes of Bernard, Wendy, and Frank illustrate the various ways that agency can be recruited in the service of resistance rather than resilience.

Bernard failed to recognize the extent to which his desire for a deeper connection with his reserved father was transformed into the resentful execution of home improvement projects similar to the ones he endured, such as “holding the flashlight” for his father while wishing his father would play with him. His admiration and affection for his father made it difficult for him to recognize and accept that his own desires were often very different from his father’s priorities.

Wendy was well aware of her desire to become a psychotherapist, although it took quite some time for her to express that desire to her own psychotherapist. She saw herself as “social” and “hard-working,” unlike her college professor sister, who was “the smart one.” Wendy accepted working as an activities director after receiving her degree, rather than seeking a clinical position. While her capacity for empathy, for instilling trust, and for negotiating complicated institutional dynamics was recognized by her coworkers and her therapist, she did not recognize it in herself. “Attuned disruption” of her attribution of her success to being merely “hard working” led to a more realistic appraisal of her skills and potential, as well as an increase in receptive affective capacity.

Frank evidently enjoyed a constitutional proclivity to undertake daunting challenges with authentic enthusiasm. His account of cooking dinner for his sisters, with both his parents offstage, did not suggest any inherent conflict for him. His sense of mastery was so salient that his parents’ emotional distance did not register, and “earning his keep,” like Wendy’s (“I’m just a hard worker),” were ego syntonic rationalizations that required “attuned disruption.” Frank’s relationship with his wife activated long-suppressed relational needs. The therapist’s observation that the love that he and his wife expressed to each other was not earned by action, but that those actions were motivated by love and generosity. His recognition of his own generosity, as well as fear of loss and craving for acknowledgment, made it possible for him to recognize that his
wife’s love for him was not predicated on or maintained by his frantic “earning his keep.”

It is not a coincidence that the term “recognition” appears of each of these case vignettes. As previously noted, recognition can refer not only to the routine registration of something previously known, but also to the realization of something new that feels true – perhaps like Bollas’ (2018) “unthought known,” something that never previously recognized, but was somehow known all along. Maybe, following Taylor (1985), we should call this “strong recognition.”

1. Agency is purposeful action, a characteristic of living organisms. Agency in other animals, even other social animals, is a relatively uncomplicated expression of resilience. Living organisms differentiate between what is beneficial and what is harmful. They are genetically (and epigenetically) endowed with agentic potentialities and capabilities that are matched to the affordances of the environmental niches that have co-evolved with them. For social animals, other than us, safety was provided by collective existence, living in groups with relatively stable social hierarchies.

As human evolved we increasingly adapted our environments to ourselves, rather than adapt to them, ultimately creating a diverse range of artificial (or artifactual) physical and cultural environments. Neuroplasticity makes it possible for us to develop the agentic capabilities necessary to navigate within a wide variety of cultures Learning through anticipation and imitation expanded upon “instinctual” knowing, fostering our unique capacity for invention, innovation, and imagination. This creative potential, paired with our fundamental need for collective existence, generates an inherent tension between autonomous agency and social cohesion.

Temperamental differences and the vicissitudes of attachment may determine a particular individual’s relationship to the norms of a particular culture, as cultures vary widely in the celebration, tolerance, or censure of different expressions of autonomy. Even within a particular culture, we exist in multiple subgroups, each with their own status hierarchies and norms. So human agency is a very complicated matter.

2. It has been argued that human agency is manifested in various ways, not only in the service of autonomy but also in adjudicating the tension between self-interest and accountability to others—the yin and yang of social existence. The balance between individualism and collectivism, between independence from and solidarity with others, is negotiated in different social contexts and within each individual in every culture. Resistant agency can obscure authentic desires and distort the evaluation of capabilities and accountabilities. Resilient agency promotes an adaptive balance between the individual and the culture in which the individual exists. Resilient agency includes attunement to biological and psychological needs and desires, as well as both habits and conscious decisions. Over the course of the human life cycle, transformative agency entails the recognition of new desires, the expansion of capabilities, and the re-evaluation of accountabilities as we grow up and grow old.

4. Like agency, transformance, too, is a many-splendored thing. Eileen Russell equates it with “surrender, peak experiences, flow, self-actualization, and flourishing” (2015, p. 35). Diana Fosha (2008, p. 292), in her initial formulation of the term transformance, described it as “an overarching motivational force, operating in both development and therapy.” She explicitly credited Emmanuel Ghent for proposing that “surrender” could be the opposite of “resistance,” but noted that “transformance” had a broader connotation, referring to development in general, not merely the relinquishing of defenses. Benjamin (2018) uses the term “surrender” to describe the shift from complementarity to mutuality, from a dyadic configuration of dominance and submission to the acknowledgment of the agency of the other without relinquishing one’s own agency. While Ghent’s use of surrender might be seen as limited to the psychotherapeutic change from resistant to resilient agency, Benjamin’s surrender is a developmental, not exclusively psychotherapeutic, achievement. Surrender is receptivity and recognition of something both new and true.

5. Many paragraphs ago, it was observed that agency is often conflated with autonomy and activity. Its function in the service of relational safety and its receptive aspect were both often overlooked. Benjamin’s understanding of surrender suggests that the development of agency requires not only the active exercise of agency but the surrender of agency as well. The achievement of mutuality that Benjamin outlines is a paradigmatic instance of transformance. Benjamin’s surrender is not submission, and agency is retained, not relinquished, and recognized both by and in the other person as a “like subject” rather than an object. Experience of the self now includes awareness of the experience of the other. If resilience is action on behalf of the self, transformance may be, among other things, an expansion of the experience of the self.

6. What is the relationship between recognition and agency? Benjamin’s (2002) discussion of Louis Sander’s notion of “the rhythm of recognition” between infant and caregiver describes an alternation of reactivity to the caregiver and then a “creative disengagement… in a state of equilibrium,” in which the infant experiences a sense of autonomy—not “absolute autonomy” but rather agency within an interactive system (p. 45)—a nascent mutuality “like the rhythmic structure or pattern that two or more partners simultaneously create and surrender to” (p. 49). Mutual recognition involves not only recognizing the other but also recognizing that the other is recognizing you. Psychological change, the reorganization of schemas of beliefs, emotions, and expectations, occurs when we experience, if we can borrow Piaget’s terminology, a “disequilibrium”—when the assimilatory assumptions can no longer be applied (Wachtel, 1981). This is a disruptive recognition, a new and unfamiliar experience. It requires surrender, making room for something new, either relinquishing or modifying one’s experience: a reaction, followed by an internal reorganization. Benjamin argues that “the concept of recognition entails the idea that having an impact on the other directly affects one’s own sense of agency” (2002, p. 45). Recognition in this sense is not simply the registration of something previously known but a realization, an active and expansive receptivity. The strong recognition that is a manifestation of transformance is the realization of something new and something true—an unconditional and unconventional acceptance.

Attuned disruption of the patient’s fear-based expectations of others may occur when the therapist meets the patient with both protective authority and compassion, with the therapist’s own surrender to an appropriate degree of emotional openness and self-disclosure. This may make it safe for the patient to likewise surrender to mutuality and greater receptive affective capacity, to accept a degree of dependence without relinquishing a sense of autonomy. Karen Maroda, way back in 1999, noted that “although the literature on the topic of

surrender, either unilateral or mutual, may be almost nonexistent” (p. 53), the surrender of the therapist as well as the patient was crucial, and that “we communicate to the patient that we have shared his or her emotional experience and ‘emerged intact,’ ” not merely “containing” the patient’s experience, “calm, in control, and above the fray” (p.55). Metaprocessing both the patient’s emotional experience and the patient’s emotional experience of the therapist’s emotional experience of the patient’s emotional experience can build receptive agency and may be an essential prerequisite for more active expressions of agency and transformance.

8. Transformance strivings expressed by the decision to engage in a psychotherapeutic relationship are quickened by the therapist’s receptivity. Utilizing Benjamin’s terminology, the therapist’s initial “meeting” is succeeded by “marking,” which incorporates empathic attunement and cognitive differentiation. The safety of the therapeutic relationship elicits greater receptivity and the possibility of the disruption of resistant agency. The therapist as True Other can recognize the patient’s previously unrecognized desires and capabilities and provoke the reassessment of accountabilities, allowing the patient to receive recognition in a new way.

9. Jeremy Safran’s meditation on agency and surrender (2016) noted that the Taoist concept of wu-wei was often translated (or perhaps mistranslated) as “nonaction” but did not imply passivity. He noted Ghent’s observation that Western individualistic cultural biases might obscure the distinction. Safran related surrender to gratitude and grace, to unconditional acceptance. Surrender is purposeful, a manifestation of receptive agency, what Erich Fromm (2016) calls “inner action.” Danny Yeung (2024, p. 28) describes a process of surrendering a preoccupation with presence in order to experience presence, an “effortful emptying” that “results in the effortless emergence of presence.” Recognizing and relinquishing our intention in order to receive—perhaps like exhaling fully with intention to allow your lungs to be refilled without effort.

10. Agency is purposeful action, whether in the service of safety or autonomy, expressed in resistant, resilient, and transformational action. But agency also has a receptive aspect, a surrender to create a space that can be filled by something new. Transformational agency involves deeper awareness of both the embodied self and what is beyond the boundary of the self. It is activity and receptivity, autonomy and

accountability, generosity and gratitude, mastery and limitation, pride and awe. It is the satisfaction and the surrender of desire. It is will and willingness.

The Editor’s Letter

The Core-Self issue offers four insightful articles, and a poem, exploring how AEDP facilitates the emergence of the flourishing core-self. The contributions examine this emergence within the contexts of development, healing, and, in Jacquie Ye-Perman’s article, broader cultural dynamics. 

Both Ben Susswein and Annika Medbo draw inspiration from Eileen Russell’s (2021) work on the agentic self. The issue begins with Ben Susswein’s extensively researched article on the development and expression of human agency within attachment relationships. Susswein distinguishes resilient agency from resistant agency and incorporates Russell’s concept of “attuned disruption” alongside recognition processes to address and repair resistant agency. His exploration lays a foundational understanding of the role agency plays in flourishing. 

We follow Susswein’s erudition on agency with Annika Medbo’s innovative approach on working with patients who have not yet developed an agentic core-self. She begins therapeutic work with these patients by joining them in their State 2 maladaptive state of ‘no-self’ and creates a relational bridge to adaptive State 2 functioning. Medbo’s interventions are represented by her wo new triangles of experience: The Triangle of Finding and Rescuing the Self and The Triangle of the Emerging Self. 

My article follows Medbo’s, delving into an ecological model of recognition as central to the development of a flourishing core-self and to the therapeutic resolution of core-self hindrances. I highlight the role of core-state experiences that emerge after pivotal moments of therapeutic recognition and demonstrate how these experiences reinforce the flourishing core-self. 

Finally, Jacquie Ye-Perman continues the discussion of core-state, reframing it not as an endpoint but as a state of readiness for deeper trauma work. Her clinical case study focuses on a Chinese client, illustrating AEDP’s cross-cultural applicability through its emphasis on affect and somatic processing. Ye-Perman highlights how core-state experiences set the stage for transformative growth and resilience, even in the context of profound trauma. I close this letter with a poem by Gerald Brooks, a beautiful encapsulation of the core themes explored in this issue’s articles. It is also a prelude to Brook’s work to be featured in an upcoming issue of Transformance.

Dear Colleagues,
I am delighted to present a preview – in the form of a poem – of Gerald Brooks’ 1 forthcoming article, The Echo of Absence: Father Hunger, the Inner Child, and the Weight of Silence which will be published in Transformance: The AEDP Journal, Volume 16, Issue 1, in early 2026.

Gerald’s scholarly work on incorporating Nigrescence Theory (Black identity development) into AEDP practice is a significant contribution to AEDP’s efforts to expand the therapeutic applications of trauma treatment beyond the family unit and into systemic societal traumas, such as racism. He has presented his approach to working with Black clients, as a Black therapist, in Immersion and Advanced Skills courses, in presentations titled, Unshackled Souls: A Braided Journey to Core State, Collective Liberation and Collective Empowerment.

He will be presenting Black Men Do Cry: The Transformational Tear, Father Wounds, and the Healing Power of AEDP at the April 2025 AEDP conference in NYC. Gerald is not only a talented and deeply thoughtful therapist but also a gifted poet. Below is his poem, Father Hunger, which we are proud to present in advance of his forthcoming publication in Transformance: The AEDP Journal.

Article One: Agency is resilience in action: The role of recognition in transforming resistant agency to resilient agency

Abstract: The concept of agency has recently been introduced into the conversation about attachment-based psychotherapy. Agency can be a manifestation of resilience, the intrinsic capacity of all living organisms to differentiate between what is beneficial and what is harmful. Although human agency is often equated with the expression of autonomous action, it also functions in the background to provide the relational safety required for autonomous functioning through receptivity and mutual recognition. Relational safety begins with attachment and evolves into varied forms of affiliation that provide security and entail the obligations and accountability that constrain autonomy. The resilient “self-at-best” might be characterized as a state of optimal balance between autonomy and accountability. Agency can also serve a “resistant” rather than resilient function for individuals who do not experience sufficient relational safety, operating in the service of a “false self” or the “self-at-worst.” Psychotherapeutic interventions can restore an adaptive balance between autonomy and relational safety. The relational safety that attachment-based treatment provides is necessary, but it is not a sufficient for healing when a client’s agency is compromised – when agency is resistant, inhibited or misdirected, and not resilient. “Attuned disruption” of resistant agency may be necessary to restore and support the resilient function of agency and promote transformance. Case vignettes illustrate the resistant use of agency in misdirecting desire, disguising capability, and sacrificing growth to preserve a sense of relational safety. Active and receptive modes of agency are considered with regard to the importance of mutual recognition in resilience and transformance.

Article two:  Finding, Forming and Transforming the Self: A Journey From No Self to Core Self

Abstract: This article presents my work with a patient whose sense of Self was profoundly unformed, necessitating extensive engagement within the realm of State 2 maladaptive affective experience. An AEDP’s guiding ethos – to create our interventions, moment-to- moment, by tracking the unfolding phenomenology – allowed me to innovate within the AEDP framework. In the course of my work with this client I devised two new triangles of experience: the Triangle of Finding and Rescuing the Self, and the Triangle of The Emerging Self. These triangles offer a conceptual visualization of the work primarily happening within maladaptive State 2 affective experience and in the process of moving from maladaptive affective experience into adaptive affective experience. 

The theoretical framework in this article builds on Eileen Russell’s (2021) incorporation of agency, will, and desire as State 2 affective experiences. She proposes the idea that “neglect, exploitation, and oppression of any kind leave holes in the development of self” (p.244), a deficit that Fosha has similarly described as unformed experience (Fosha, 2013). I propose that when working with clients who present with an unformed Self, we must connect with our patient within the realm of State 2 maladaptive affect; it is there that we encounter the neglected split-off part of the Self, needing to be rescued. As was true for my patient, the maladaptive affective experience might be the (yet) only “road” to their inner emotional life. Without being recognized, witnessed, and felt in the presence of a safe ‘other’ the split-off divided self cannot be unified, and the True Self cannot form and transform. Simply said, the patient needs to know they have a Self and have a felt sense of that Self before they can feel about and for the Self.

Article Three:  An ecology of Core Self flourishing: The predominant role of recognition in development and healing

Abstract. AEDP is rooted in ecological biology and affective neuroscience, emphasizing the inherent human propensity to flourish. The therapeutic approach assumes that the intact self, when sequestered in response to a hostile environment, can emerge when met with an attachment environment conducive to flourishing. Recognition-rich interactions create this environment and welcome the patient into it. At the core of AEDP’s framework is the concept of the Core Self, which can be divided into two dimensions: the neurobiological core self, innate and unchanging, and the experiencing core self, which adapts and evolves in response to the external environment. Recognition processes, ranging from fundamental reflexes to complex affirmations, underlie the sense of continuity and coherence for the Core-Self.

Recognition is explored from its conceptual origins in ecological biology and neuroscience to its application and elaboration by Fosha in AEDP theory and treatment. The role of recognition and benign mis-recognition in developing the agentic Core Self is examined, while pernicious mis-recognition is identified as a basis for dissociated self-states. Recognition is highlighted as a central mechanism for healing the fractured self. Recognition-rich, moment-to-moment tracking of somatic-affective experiences serves as a dual mechanism for processing trauma. Along with AEDP intra-relational interventions, it facilitates the integration of dissociated self-states. The article concludes with two case studies illustrating the effect of recognition-rich interactions to bring about core state and the felt-sense of a flourishing Core Self.

Article Four: “I can give you a bigger space” Transformation Starting from Core State: A cross cultural case study

Abstract: This article is centers on two main points illustrated through a case study of my work with a Chinese client. The first point demonstrates how AEDP techniques inherently assist cross cultural work due to its focus on phenomenology, and its recognition of universal human strivings for safety, attachment and growth – all with their matching non-verbal, emotional/physiological markers. I will demonstrate how techniques of moment-to-moment tracking and metaprocessing, intuitively translated into deep and profound transformation for my Chinese client. The second point proposes that core state is a place for therapist to remain active, harnessing the momentum of change towards further transformation. Through session transcription, I will illustrate unique aspects of the treatment when a session begins in core state, highlighting how starting in core state shapes the work and accelerates readiness for another round of deep trauma work.
